To address the alarming trend in chronic disease, ÷ÈÓ°Ö±²¥ has partnered with key community stakeholders to improve health one community at a time.


Healthy Communities is a place-based initiative that encourages residents to lead healthier lives - and provides resources to help. Its integrated framework coordinates primary healthcare, public health and community interventions; and emphasizes a shared responsibility for community health, while encouraging residents to take active roles in their own health.

340B Program Supports ÷ÈÓ°Ö±²¥'s Culture of Caring Community Partnerships

÷ÈÓ°Ö±²¥’s mission is to provide compassionate and extraordinary care every day to all patients regardless of where they live or their financial circumstances. We rely heavily on philanthropy, corporate dollars and 340B savings to deliver these services to the men, women and children in the 110+ communities we directly serve.
